Nursing visits will consist of a complete physical assessment and evaluation of the patient condition.
The skilled nurse will also instruct the patient and family/caregiver in disease process and management, medications and other pertinent topics. With case management, usually nursing visits will be frequent at first. Then as acute medical problems are resolved, and home instruction completed, visit frequencies will decrease and discharge planning and teaching will begin.
